Apprenticeship Block Release Enrolment Form 2013-2014

Fields marked with a red star (*) are mandatory.

It is important to register for training as soon as possible as seats are given on a first come/first served basis. Forms must be returned before your name will be put on a list for training.
In order to attend block release training and have your training paid for by SkillsPEI you must::


1. Be a registered apprentice with the PEI Apprenticeship Section, Department of Innovation and Advanced Learning.
2. Be eligible for Employment Insurance.
3. Be unemployed at the beginning of training.
Important Notice:


Once submitted, your name will be added to the course list and you will receive your training notice and forms in the mail 3 to 4 weeks before the start date of the course
