Living a Healthy Life

Living a Healthy Life is a fun and practical program that helps people with ongoing health conditions overcome daily challenges and maintain active, fulfilling lives.  Throughout the program, people develop the skills they need to help themselves. They gain confidence and motivation to manage their health, and feel more positive about their lives.

 What is the Living a Healthy Life Program?

Living a Healthy Life is a 6-week chronic disease self-management program.  Sessions are held once a week for 2 1/2 hours, with a short break included. Participants will receive a copy of the book "Living a Healthy Life with Chronic Conditions".

 


Who is the program for?

The program is for people who have a chronic condition, their caregivers or loved ones.  Chronic conditions may include:

  • arthritis
  • cancer
  • chronic lung disease - i.e. asthma, chronic obstructive pulmonary disease (COPD)
  • chronic pain
  • diabetes
  • heart disease
  • high blood pressure
  • other chronic conditions

People who are looking to make changes to prevent health problems are also welcome.

What does the program look like?

The program is highly interactive and in a group of between 10-15 participants.  Peer support among participants is a key component. This means that participants share and learn from one another (participants only share what they are comfortable with sharing).

Setting goals (or action plans) is a big part of the Living a Healthy Life Program. This plan will help participants take small, realistic steps toward meeting their own personal goals. Participants will be encouraged to carry out the plan they set up for themselves.

Workshops are led by 2 leaders who have been trained to deliver the Living a Healthy Life program.  These leaders are volunteers and health care professionals, many of whom have chronic conditions themselves or have someone close to them with a chronic condition.
