Nurse Practitioner
Full Time Permanent (PEINU) Position


Department : Health PEI
Location : O'Leary Health Centre, West Prince Primary Health Care Net., O'Leary

(COMMENCING IMMEDIATELY)

The purpose of this position is to maintain, improve, restore and optimize the health status of clients within a practice setting through direct client care for individuals, families, and communities. The Primary Health Care Nurse Practitioner functions autonomously within a collaborative practice setting while providing advanced holistic care of acute and chronic illness to clients across the lifespan. The Nurse Practitioner manages a client caseload as an integral member of a Primary Health Care Network Team. Duties will include:

- perform comprehensive, focussed health assessments including a health history and physical examination using a holistic framework and multiple assessment techniques relevant to a client’s stage of development, culture and health.

- analyse client symptoms and assessment data by applying the principles of patho physiology, pathogenesis, epidemiology, demographics and relevant research.

- determine the need, order, and interpret the results of relevant screening and diagnostic tests.

- systematically compare and contrast clinical, behavioural and diagnostic data to determine differential and working diagnosis.

- provide direct guidance and counselling on health promotion and illness prevention strategies in accordance with client health status and needs.

- participate in the overall evaluation of the practice setting’s programs and services.

- collaborate with interdisciplinary teams and community members to identify client population health needs and resources, and provide input as requested for community health programs.

- ensure professional standards and clinical competence are reflected in all educational content taught by Primary Health Care Nurse Practitioners, and are demonstrated by nursing staff in the work unit.

- performing other related duties as required.

Bi-Weekly Hours: 75.0 hours bi-weekly / days / evenings, nights and weekends as required

Qualifications :

- Must have a Masters Degree in Nursing as a Nurse Practitioner or graduation from an approved and recognized Nurse Practitioner Program with extensive experience as a registered nurse in hospital and community settings. Previous Nurse Practitioner experience in hospital and community settings is preferred.

- Must be Eligible for licensure as a Registered Nurse with the endorsement as a Nurse Practitioner with the Association of Registered Nurses of PEI.

- Critical thinking proficiency to assess, analyse, diagnose, select treatment, evaluate, and refer patients with common acute or chronic health conditions, or for health promotion / illness prevention.

- Skilled in independent performance of advanced elective procedures.

- Demonstrated ability to work effectively in an interdisciplinary environment with excellent collaborative, team building and consultative skills.

- Excellent communication (verbal and written), facilitation and interpersonal skills in dealing with clients, families, community groups, professional colleagues, and referral partners.

- Thorough knowledge and proficiency in medical and nursing physical assessment and treatment methods.

- Demonstrated ability to prepare and present information on health promotion and illness prevention to the community and practice setting staff.

- Strong analytical, problem-solving and decision-making abilities.

- Must possess a valid driver’s licence and have access to a reliable vehicle.

- The successful applicant must have a good previous work and attendance record. Other Qualifications:

- Previous community based Primary Health Care Nurse Practitioner experience preferred.

- Previous experience in leadership roles would be an asset.

- The successful applicant must provide a satisfactory criminal records check prior to beginning employment.

Note: The collaborative care model on PEI is a province-wide initiative that will involve service delivery changes in all areas of the health system including acute care, long term care, home care, and primary health care. The collaborative care model has not yet been implemented in all unit/work sites. Staffing levels may change upon its introduction and will be in accordance with respective collective agreements and union consultation.
